Imaging and other healthcare providers delivering the technical portion of advanced diagnostic imaging (ADI) services are required to obtain accreditation from a CMS-approved accrediting organization to qualify for reimbursement. It's important to note that this requirement does not apply to hospitals or critical access hospitals.
According to Section 1861(d) of the Social Security Act, an ADI supplier may include a physician, other licensed practitioner, or a facility like an Independent Diagnostic Testing Facility (IDTF) that provides the technical aspect of imaging services. The “technical component” refers to the non-physician elements of imaging delivery, such as staff time, equipment usage, and administrative functions.
Section 1834(e)(1)(B) further specifies that advanced diagnostic imaging encompasses procedures like MRI, CT scans, and nuclear medicine imaging, including PET scans. It does not cover x-rays, ultrasounds, fluoroscopy, or mammography. However, mammography—both diagnostic and screening—is regulated by the U.S. Food and Drug Administration.
Accreditation guidelines require on-site personnel to be trained to rapidly recognize, assess, diagnose, and treat adverse reactions to contrast material. This includes the ability to respond effectively during urgent medical events.

Accreditation pricing is generally based on a per-unit rate ranging from $2,200 to $3,700. Some accrediting organizations offer discounts to facilities submitting applications for more than three modalities, as well as volume-based discounts for large practices.
